Insurance I currently accept Medicare, Aetna, and United Healthcare/OPTUM insurances and submit treatment billing on your behalf. For other insurances, I can provide a monthly billing statement for out-of-network reimbursement. Please contact your insurance provider to determine your plan coverage.
Payment I accept online payments, personal check, and major credit cards as forms of payment. You are responsible to pay any copayments, deductibles, or share of cost at the time of service.
Cancellation Policy There is a late cancellation/no show fee for appointments cancelled with less than 48 hours notice.
