Insurance I accept Medicare, Aetna, and United Healthcare/OPTUM insurances and submit billing information to the insurance company on your behalf. I can provide a monthly billing statement for out-of-network reimbursement for other insurance plans. Please contact your insurance provider to determine your plan coverage and verify that I am in-network.
Private Pay/Fee For Service
In January 2022, the No Surprises Act became effective, requiring all healthcare providers to inform patients who are uninsured or plan to pay out of pocket with an estimation of healthcare charges. This estimate is part of my treatment agreement prior to the start (or continuation) of care. Payment Venmo, Zelle, FSA/HSA, and major credit cards are acceptable forms of payment. You are responsible to pay any copayments, deductibles, or share of cost at or before the time of service.
Cancellation Policy There is a late cancellation/no show fee for appointments cancelled with less than 48 hours notice.
